Patch Test/Sensitive Test Policy

Purpose

The purpose of this Patch Test Policy is to establish a clear understanding and agreement between the treatment provider IVONNE, Inc. (hereafter referred to as "IVONNE") and you the client regarding the administration of patch tests for permanent makeup, tinting, laser, and other treatments. This policy outlines the conditions under which a patch test is required, the procedure for requesting a patch test, and the responsibilities of the client and IVONNE in relation to patch testing.

Scope

This policy is applicable to all clients who are either considering or have scheduled an appointment to receive permanent makeup services at IVONNE. These services include, namely, eyebrow tattoos (such as microblading and nano brows), eyeliner tattoos, lip tattoos, and freckle tattoos. It also applies to those seeking camouflage tattoos, tinting services, and various laser procedures, namely laser hair removal, laser skin rejuvenation, and laser tattoo removal. A preliminary patch test is offered and in some cases required for any of these treatments.

Policy Statement

IVONNE is committed to the safety and satisfaction of its clients and offers a patch sensitivity test to all clients to minimize the risk of adverse reactions to products or services.

Patch Test Availability

A patch sensitivity test is available upon request for all clients prior to scheduling their treatment appointment.

Clients must request and complete a patch test no later than 72 hours before their scheduled appointment. IVONNE cannot accept any responsibility for patch testing that is requested too late, or not at all, or for cancellations or rescheduling due to incorrect use of this policy.

Patch tests are treatment appointments and are subject to availability, and it is the client's responsibility to ensure they are scheduled in a timely manner. Patch tests treatments are subject to the same contraindications and eligibility requirements as those found in the Service Agreement, Terms and Conditions, Scheduling Intake Form including Informed Consent, and other policies.

Client Acknowledgement

Clients acknowledge they have been offered the opportunity to undergo a patch test before scheduling any treatment.

A patch test is designed to minimize the risk of adverse reactions but cannot guarantee that such reactions will not occur.

Clients must confirm they have either:
a. Completed a patch test and confirm that there have been no adverse effects and that they are satisfied with the results, or
b. Chosen to waive the patch test opportunity.

By proceeding with scheduling or completing a treatment appointment you agree that you have confirmed that IVONNE has met its obligations with regards to patch testing. Clicking on the “I agree” box, or "Complete Appointment" or "Make Payment", or "Pay Now" in the scheduling workflow is effectively one and the same as a digital signature, and this memorializes intentions to be bound to the terms of this agreement and the Patch Test Policy. Should a staff member, or agent of IVONNE schedule an appointment for you on your behalf this shall have the same effect as your acknowledgment and agreement.

Patch Test Waiver and Liability

Client's Assumption of Risk

Clients who elect to proceed without a patch test (hereby referred to as a "waiver") knowingly and willingly assume all risks associated with the treatment. This includes both known and unknown potential adverse reactions that may result from the treatment. Likewise, clients who opt to receive a patch test also acknowledge that this is a preliminary measure to test for sensitivities and that adverse reactions may still occur as a result of the patch test itself.

Financial Responsibility

Clients are responsible for the costs associated with any patch test treatment. However, at the sole discretion of IVONNE this cost may be applied as a credit to the subsequent scheduled treatment appointment. This is in recognition of the exploratory nature of the patch test and its role in ensuring the compatibility of the treatment with the client's body.

Limitation of Liability

IVONNE explicitly disclaims responsibility for any adverse effects that may occur as a result of the client's decision to decline the patch test, as well as for any adverse reactions that may occur as a result of undergoing the patch test. The decision to waive the patch test or proceed with it is the sole responsibility of the client, and by doing so, the client releases IVONNE from any claims, liabilities, or damages that may arise from the treatment or the patch test itself.

Clients who waive the patch test or undergo the patch test understand that they are either foregoing or receiving an exploratory treatment designed to determine compatibility with the treatment materials. By proceeding with or without the patch test, the client accepts the potential for adverse reactions and agrees that IVONNE will not be held liable for such reactions, regardless of whether they occur during the patch test or as a result of the subsequent treatment.

Clients are advised that a successful patch test does not guarantee the elimination of any risks associated with the treatment.

Recertification of Patch Test

  • Clients will be required to undergo a new patch test if:
  • The related equipment at IVONNE undergoes replacement, maintenance, or service that may affect treatment outcomes.
  • The laser technician performing the treatment has received specific instructions from the manufacturer or supplier to conduct a patch test.
  • A new area of the body is scheduled to be treated that was not previously tested.

Compliance

Clients are expected to comply with this Patch Test Policy as a condition for receiving treatment.

Non-compliance with the patch test requirements may result in the cancellation of the scheduled treatment without any refund or further obligation on IVONNE.

Procedure for Patch Test

Clients must request a patch test by contacting IVONNE at least 72 hours before the initial treatment.

IVONNE will schedule the patch test based on availability.

Both IVONNE and the client are responsible for communicating the results of the patch test, and further instructions and scheduling eligibility will be determined based on the outcome.

Acknowledgment

By scheduling a treatment, clients acknowledge they have read, understood, and agreed to the terms and conditions outlined in this Patch Test Policy.

Changes To This Policy

IVONNE reserves the right, but not the obligation to amend this policy from time to time, and all without notice. It is your responsibility to visit our website and to stay apprised of our policies as they may change from time to time. Due to the nature of email communication being undelivered or sent to SPAM there is no guarantee that our notices will reach you.

Copyright Notice

The content, including text, graphics, images, and information contained in this Patch Test Policy, is the property of IVONNE ("the Clinic") and is protected by copyright laws. Unauthorized use, reproduction, or distribution of this policy, in whole or in part, without express written consent from IVONNE is strictly prohibited.

Limited Use

This Patch Test Policy is specifically designed for use within IVONNE and is not applicable to any other clinic, provider, or entity. IVONNE accepts no responsibility for the consequences of this policy's use by any other clinic or provider. This policy is not intended to be used as a standard or guideline for any other institution or practice.

Infringement Notification

Any unauthorized use of this policy may constitute a violation of copyright laws. If you become aware of any such infringement or unauthorized use, please report it immediately to legal@ivonne.ca. IVONNE reserves the right to take legal action against any individual or entity found to be in violation of these terms.
